Dr. Alice Bonner, Secretary for the Executive Office of Elder Affairs, will be the key note speaker at an event being held at Regis College. The program, entitled “Current Trends in Improving Care Transitions,” is intended for health care professionals and nursing students who want to learn more about strategies to help decrease hospital readmissions. Participants will hear about the highly successful Transitional Care Program at Southwestern Vermont Medical Center. They will also learn about the expanding Care Transitions Education Project aimed at uniting nursing students and nurses from a variety of care settings. This program will include continental breakfast, lunch, and contact hours.

                             Date: Friday November 10, 2017

                             Time: 8:00 – 4:00 PM (registration begins at 7:30 AM)

Place: Upper Student Center Lounge, Regis College  

           235 Wellesley St. Weston MA 02493

Cost:   $119 for healthcare professionals

             $79  for nursing students
